Emergency Dental ("we," "us," "our") operates dental practices in Indianapolis, Greenfield, and Bloomington, Indiana. This policy explains what information we collect when you use our website or visit one of our locations, and how we use, share, and protect it.
When you hold a spot online, contact us, or fill out a referral form, we collect information like your name, phone number, email address, preferred location, and arrival time. If you become a patient, your care involves protected health information (PHI) handled under HIPAA. See our Notice of Privacy Practices, available at any location, for how that information is used.
We use the information you give us to hold your place in line, confirm your visit by text message, staff our locations appropriately, and follow up on referrals from other providers. We do not sell your personal information.
By providing your phone number when holding a spot, you consent to receive a one-time text confirming your arrival time. Message and data rates may apply. You are not enrolled in any recurring text program.
We share information only with the location you're visiting, referring providers when relevant to your care, and vendors who help us operate (such as our scheduling and messaging systems), each bound to protect it. We disclose information when required by law.
You can ask any location to review, correct, or delete the contact information we hold for you that isn't part of your clinical record. To request changes to your clinical record, contact the location where you were treated directly.
